Noon – Abdominal discomfort started, bloated feel. Midnight – pain worsened. 3am sent to hospital – diagnosed with appendicitis.

Severe back pain, abdominal pain persist

Blood test confirm infection, ultrasound shows no collection – warded

Antibiotic treatment – Severe backpain & abdominal pain persist

3pm

Abdominal CT scan (oral & rectal contrast used).

Radiologist report read (part)

There a collection medial to the caecal pole measuring 5.0 x 6.5cm in diameter and 6.0cm in height. There are contrast and pockets of  air seen within.

Radiologist IMPRESSION

Post operative collection medial to the caecal pole. Contrast and pockets of air seen within.

Likely infected  collection, leaking from the appendicular stump.

Surgeon decided that immediate surgery necessary.

8pm

Laparotomy surgery

Post surgery note – patient woken up immediately after surgery in severe wound pain – without painkiller. Patient was pushed to his ward still in pain. Painkiller was given about 30 minutes after patient was sent to ward.

Post surgery note – surgeon briefed patient’s wife that he discovered that the appendicular stump stitches was loose, and that has been causing internal leakage and infection. He further noted that this was his first case of such leakage since he started practice as a surgeon since 1985.

Antibiotic treatment.

Patient was given high dosage of painkiller (on request) – low dosage has no effect on severe surgery wound pain. Side-effect – nausea & vomitting.

13/4/2011

Painkiller dosage reduced to minimum as wound pain lessen.

Due to frequent vomitting, nurse advised for stomach tube insertion to empty the stomach. Patient agreed to proceed. 30min after insertion, patient vomitted the entire stomach content. Patient requested the tube to be removed soon after.

Night, patient requested to stop the painkiller as he can endure the pain and didn’t want to continue having nausea.

17/4/2011

As surgeon instructed, nurse removed the main tube for liquid discharge. Against surgeon instruction, nurse removed the secondary small tube for liquid discharge.

Patient was discharged from hospital.

Surgeon has waived his surgery fee. Hospital and anaestheologist fee were charge at regular rates.

20/4/2011

Follow-up visit to surgeon. Due to consistent amount of liquid discharge from the infection, surgeon suggested to insert back the small tube for more efficient flow. Surgeon admitted that the nurse did not follow his instruction. Patient request time to decide.

22/4/2011

Patient went to Selayang hospital to get 2nd opinion. Upon requesting a referal letter from the surgeon, he convinced the patient to come back to him for further treatment, and will not charge for his service. Patient went back to the surgeon to get the tube inserted back in.

30/4/2011

Follow-up visit. Surgeon decided to let the tube in for another week.

6/5/2011

Follow-up visit. Tube was removed.

Patient lost 5kg in 1 week of the second surgery.
